Support Broker

Kavod Healthcare System LLC is looking for a dedicated and compassionate Support Broker to join our team. The ideal candidate will have strong interpersonal and problem-solving skills, with a commitment to helping individuals navigate healthcare systems and access essential services. As a Support Broker, you will be responsible for advocating on behalf of patients, guiding them through care plans, and connecting them with appropriate resources. You will work closely with healthcare professionals, families, and community organizations to ensure the best possible outcomes for patients. A background in case management, social services, or a related field is preferred, and a passion for improving lives is essential. It is a P/T Position with a F/T prospect.

Qualifications:
1. At least One years’ experience as a Support Coordinator (NJ DDD)
2. Bilingual (Spanish/English) is a plus
2. Should be able to service participants in Essex, Bergen etc NJ areas.
3. Able to use iRecord data base
4. Have good writing and Communication skills
5. Must have references
6. Must be able to work on 10 cases at minimum
7. Own car/clean record
8. Must have a Bachelor’s Degree
9. Must be available during the days for meetings
10. ⁠Must complete the Orientation to Supports Brokerage – an online training
